ABSTRACT

Platycodonis Radix is the dry root of Platycodon grandiflorum of Campanulaceae, which has a variety of pharmacological effects and is a commonly used bulk Chinese medicine. In this study, the chloroplast genome sequences of six P. grandiflorum from different producing areas has been sequenced with Illumina HiSeq X Ten platform. The specific DNA barcodes were screened, and the germplasm resources and genetic diversity were analyzed according to the specific barcodes. The total length of the chloroplast genome of 6 P. grandiflorum samples was 172 260-172 275 bp, and all chloroplast genomes showed a typical circular tetrad structure and encoded 141 genes. The comparative genomics analysis and results of amplification efficiency demonstrated that trnG-UCC and ndhG_ndhF were the potential specific DNA barcodes for identification the germplasm resources of P. grandiflorum. A total of 305 P. grandiflorum samples were collected from 15 production areas in 9 provinces, for which the fragments of trnG-UCC and ndhG_ndhF were amplificated and the sequences were analyzed. The results showed that trnG-UCC and ndhG_ndhF have 5 and 11 mutation sites, respectively, and 5 and 7 haplotypes were identified, respectively. The combined analysis of the two sequences formed 13 haplotypes (named Hap1-Hap13), and Hap4 is the main genotype, followed by Hap1. The unique haplotypes possessed by the three producing areas can be used as DNA molecular tags in this area to distinguish from the germplasm resources of P. grandiflorum from other areas. The haplotype diversity, nucleotide diversity and genetic distance were 0.94, 4.79×10-3 and 0.000 0-0.020 3, respectively, suggesting that the genetic diversity was abundant and intraspecific kinship was relatively close. This study laid a foundation for the identification of P. grandiflorum, the protection and utilization of germplasm resources, and molecular breeding.

ABSTRACT

Gastroesophageal reflux disease (GERD) is one of the most common digestive diseases with high incidence, complicated clinical symptoms, difficulties in standard treatment, and heavy medical burden. At present, some GERD-relevant clinical practice guidelines (CPGs) have been issued by different countries and academic organizations, but some recommendations were inconsistent, which has caused some problems for the current clinical whole-course management of GERD. To summarize the relevant evidence among the CPGs on GERD and formulate the whole- course management strategies, we included GERD-relevant CPGs published or updated after 2010 by searching websites of guidelines, relevant professional societies, and electronic databases. We extracted the recommendations and summarized the evidence from the aspects of symptoms, epidemiology, diagnosis and treatment, which was presented in the form of evidence mapping. We included 24 CPGs, including three in Chinese and 21 in English. The clinical practice management strategies of GERD were formulated based on the evidence from the aspects of clinical symptoms, diagnostic methods, medical treatment, anti-reflux surgery and endoscopic treatment, psychological treatment, and traditional Chinese medicine treatment.

ABSTRACT

Objective To evaluate the performance of contrast-enhanced ultrasound (CEUS) liver imaging reporting and data system (LI-RADS) LR-5 in the diagnosis of hepatocellular carcinoma (HCC). Methods The clinical research reports with the application of CEUS LI-RADS in the diagnosis of HCC were collected from PubMed,Embase,Cochrane Library,CNKI,and Wanfang Data from inception to November 14,2021.Two researchers respectively screened the literature and extracted relevant information.The Quality Assessment of Diagnostic Accuracy Studies (QUADAS) was used to evaluate the quality of all the included articles.RevMan 5.4,Meta disc 1.4,and Stata 16.0 were employed to analyze the diagnostic performance of LR-5 for HCC in high-risk patients. Results Twenty original studies were included,involving a total of 6131 lesions,of which 5142 were HCC.The results of meta-analysis showed that the LR-5 in CEUS LI-RADS for diagnosing HCC in the high-risk population had the overall sensitivity of 0.72 (95%CI=0.66-0.77),the overall specificity of 0.93 (95%CI=0.87-0.96),the overall positive likelihood ratio of 9.89 (95%CI=5.31-18.41),the overall negative likelihood ratio of 0.30 (95%CI=0.25-0.37),and the area under the summary receiver operating characteristic curve of 0.88 (95%CI=0.85-0.91).There was heterogeneity among the included studies (I2=95.31,P<0.001).The funnel plot indicated the existence of publication bias (P=0.04). Conclusion The CEUS LI-RADS can effectively diagnose HCC in high-risk patients based on the LR-5 criteria.

ABSTRACT

Generation of mutants with clustered regularly interspaced short palindromic repeats (CRISPR)/CRISPR-associated protein 9 (Cas9) is commonly carried out in fish species by co-injecting a mixture of Cas9 messenger RNA (mRNA) or protein and transcribed guide RNA (gRNA). However, the appropriate expression system to produce functional gRNAs in fish embryos and cells is rarely present. In this study, we employed a poly-transfer RNA (tRNA)-gRNA (PTG) system driven by cytomegalovirus (CMV) promoter to target the medaka (Oryzias latipes) endogenous gene tyrosinase(tyr) or paired box 6.1 (pax6.1) and illustrated its function in a medaka cell line and embryos. The PTG system was combined with the CRISPR/Cas9 system under high levels of promoter to successfully induce gene editing in medaka. This is a valuable step forward in potential application of the CRISPR/Cas9 system in medaka and other teleosts.

BR55 is an ultrasound contrast agent targeting vascular endothelial growth factor receptor 2,which can be used to detect tumor neovascularization and improve the diagnostic accuracy.Overseas researchers have used BR55 for human ultrasound molecular imaging,which showed good safety and tolerance.We reviewed the research progress on BR55 applied in the evaluation of tumor neovascularization from the composition,characteristics,animal experiments,and clinical studies of BR55.

ABSTRACT

This study aimed to analyze the methodological quality of systematic reviews of Shuxuening Injection and evaluate the efficacy and adverse reactions of Shuxuening Injection in the treatment of different diseases,in order to provide supportive evidence for clinical practice. Three Chinese databases and three English databases were retrieved to identify systematic reviews and Meta-analysis on the efficacy and safety of Shuxuening Injection in the treatment of diseases. The AMSTAR 2( a measurement tool to assess systematic reviews 2) tool was used to evaluate the methodological quality of the included systematic reviews,and tables were created to present the results of Meta-analyses. Twenty-four systematic reviews were included,all with very low methodological quality. Among the 16 AMSTAR-2 items,only 5 items had a compliance rate greater than 60. 0%,and 8 items had a compliance rate less than 50. 0%. For patients with cerebral infarction,Shuxuening Injection combined with conventional treatment was more effective than conventional treatment alone in terms of clinical efficiency and neurological deficit improvement. For patients with angina pectoris,Shuxuening Injection was superior to Danshen/Compound Danshen Injection in terms of the total effective rate of angina pectoris and total effective rate of ECG. The efficacy of Shuxuening Injection combined with conventional treatment is significantly better than conventional treatment.Shuxuening Injection( alone or combined with conventional treatments) was better than conventional treatments for cerebral hemorrhage,ischemic cerebrovascular disease,chronic pulmonary heart disease,vertigo and sudden deafness. Shuxuening Injection had better efficacy and lower incidence of adverse reactions,but the methodological quality of included systematic reviews was low. The results of this study still need to be verified by high-quality systematic reviews.

ABSTRACT

Qingkailing Injection is one of the most commonly used traditional Chinese medicine injections with significant clinical application for the treatment of multiple diseases. This study aims to analyze the systematic reviews( SRs) of Qingkailing Injection,in order to provide reference for the clinical application of Qingkailing Injection and the development of relevant clinical practice guidelines. We searched CNKI,CBM,Wanfang,VIP,Pub Med,Cochrane Library and EMbase to collect SRs from the time of database establishment to August 2020. The eligible SRs were included according to the inclusion and exclusion criteria. AMSTAR 2 was used to assess the methodological quality. The diseases,drugs in combinations and results were extracted and analyzed. A total of 24 SRs were selected,including 10 for the treatment of acute cerebrovascular diseases,9 for respiratory infections,2 for viral hepatitis,1 for chronic obstructive pulmonary disease,and two for the adverse effects of Qingkailing Injection. Only three entries of AMSTAR 2 item were fully reported by over 70%,and the rest were reported by less than 70%,with no report about item 2,3 and 10. Twenty-nine outcome indicators were correlated with the included SRs,of which three mostly frequent outcomes were effectiveness,adverse reaction,and neurological deficit scores,showing a good efficacy of Qingkailing Injection. The common severe adverse reaction was anaphylaxis,and mild adverse reactions were skin and mucous membrane reactions. The most frequently combined drug was antibiotics,mainly Penicillin and Penicillin+Pioneeromycin. The existing evidences showed that the methodological quality of SRs of Qingkailing Injection needed to be improved and Qingkailing Injection had an obvious efficacy. However,the selection of outcome indicators for clinical trials and SRs shall be standardized,and the reporting of basic information,such as drug combination,shall be strengthened to provide more powerful clinical services.

ABSTRACT

Coronavirus disease 2019(COVID-19) is a newly emerged and highly contagious respiratory disease. Traditional Chinese medicine(TCM) has both systematism theory knowledge and clinical practical value in the prevention and treatment of COVID-19. Therefore, it was particularly important to examine the effect of TCM in the prevention and treatment of COVID-19. The patents of TCM might reflect the latest progression of scientific research. We aimed to provide reference for the prevention and treatment of COVID-19 by extracting and analyzing the TCM patents from the Patent Information Sharing Platform of COVID-19. The antiviral TCM patents were screened and exported from the Patent Information Sharing Platform. VOSviewer 1.6.14 was used to visualize and analyze the network of TCM in these patents. There were total 292 TCM patents, including 52 patents for etiological treatment and 240 patents for symptomatic treatment. Thirty-two provinces and 1 076 inventors were involved, mainly from Beijing, Guangdong and Jiangsu. Overall, there were 356 TCMs, 71 single prescriptions, and 221 compound prescriptions. The patents for treatment of coronavirus mainly focused on the treatment of coronavirus, while the patents for symptomatic treatment mainly focuses on the improvement of respiratory symptoms, such as fever and cough. There were 14 highly frequently used TCMs, including Glycyrrhizae Radix et Rhizoma, Scutellariae Radix, Lonicerae Japonicae Flos, Forsythiae Fructus, Isatidis Radix, Astragali Radix, Menthae Haplocalycis Herba, Gypsum Fibrosum, Houttuyniae Herba, Isatidis Folium, Rhei Radix et Rhizoma, Gardeniae Fructus, Platycodonis Radix, Armeniacae Semen Amarum. The analyzed results of the TCM patents from the patent information sharing platform of COVID-19 were consistent with the Guideline of Diagnosis and Treatment of COVID-19(7th edition), and the combination of TCM in each cluster may also provide future directions for drug compatibility.

ABSTRACT

Objective: To investigate the incidence, prognostic value, and risk factors associated with progression of disease within 24 months (POD24) in patients with follicular lymphoma (FL). Methods: In Tianjin Medical University Cancer Institute and Hospital from January 2010 to December 2015,the clinical and follow-up data of 140 patients who were newly diagnosed with FL and received chemotherapy was collected.The expression levels of Myc, Bcl-2, Bcl-6, P53, CD10,MUM-1, and Ki-67 in tumor tissues from 75 patients before treatment were assessed with immunohistochemical (IHC) staining. Kaplan-Meier method and Cox regression model were applied to evaluate the effect of POD24 on the prognosis of FL. Logistic analysis was performed to identify the risk factors associated with POD24. Results: Among the 140 patients newly diagnosed with FL, 42 (30%) patients showed POD24. The results of Kaplan-Meier analysis revealed that patients with POD24 showed significantly reduced overall survival (OS) compared with patients without POD24 (Pupper limit of normal, high Ki-67 expression, and positive MUM-1 expression were risk factors associated with POD24 (all Pupper limit of normal, high Ki-67 expression, and positive MUM-1 expression were risk factors associated with POD24. Moreover, FLIPI ≥3 and positive MUM-1 expression were independent risk factors associated with POD24. The sensitivity and accuracy of FLIPI combined with MUM-1 (FLIPI-M) in predicting POD24 were higher than that of FLIPI alone, and thus have high value in clinical practice.

ABSTRACT

Follicular lymphoma (FL) is a disease that features strong biological, clinical, and prognostic heterogeneity and is affected by various factors. The historically used prognostic factors such as those involved in the Follicular Lymphoma International Prognostic Index (FLIPI) and FLIPI-2, which are broadly based on clinical and laboratory features at the time of diagnosis, are of limited value. With the development of molecular imaging and gene sequencing technology, the prognostic value of the metabolic tumor burden parameters and molecular biological features (e.g., characteristics of the tumor microenvironment and epigenetic deregulation) in FL have been gradually recognized as promising, particularly in combination with the well-established clinically based prognostic indexes such as m7-FLIPI, a clinicogenetic risk model that includes the mutation status of seven genes. In addition to the baseline prognostic factors, the prognostic value of response to therapy has also been a hot topic in recent years. This review discusses the prognostic factors that affect patients with FL, referring to histological grade, metabolic tumor burden, clinically based prognostic indexes, tumor microenvironment, response to therapy, and molecular genetic characteristics, according to domestic and abroad research progress.

ABSTRACT

Bosworth ankle fracture-dislocation is rare, known to be an irreducible type of ankle injury, with a high incidence of complication. We present two cases of even rarer variants of Bosworth ankle fracture-dislocation. The first case is a type of supination external rotation adduction, and the second case is a type of supination external rotation adduction. These types have not been described before. In both of the cases we failed to achieve close reduction, and therefore proceeded with emergency surgeries, with open reduction and internal fixation. Both of the cases were performed with a postero-lateral approach to reduce the dislocations, and fix the fractures successfully. Unfortunately in one of the cases, acute compartment syndrome developed post-surgically. However, both cases showed good functional outcomes.

ABSTRACT

PURPOSE@#Undisplaced subtle ligamentous Lisfranc injuries are easy to miss or underestimate, and many cases are treated without surgical fixation. It has not yet widely known whether conservative treatment for undisplaced subtle ligamentous Lisfranc injuries may lead to a poor outcome. The purpose of this study is to compare the outcomes of conservative versus surgical management (percutaneous position screw) of undisplaced subtle ligamentous Lisfranc injury.@*METHODS@#We analysed 61 cases in this retrospective study, including 38 males and 23 females. Forty-one patients were managed conservatively, while 20 patients received surgical treatment involving minimal invasive percutaneous position screw. American orthopaedic foot &ankle society (AOFAS), foot function index (FFI, including FFI disability, FFI pain score and activity limitation scale) scores, Maryland foot score and short form-36 (SF-36) were recorded and compared after a follow-up of 10-16 months (average 12.3).@*RESULTS@#Patients in the surgical management group had higher scores in all evaluation methods (p < 0.05). The complications in the conservative management group had higher incidence, mainly including secondary diastasis (34.1% vs. 5.0%), joint stiffness after 3 months (82.9% vs. 0%), and secondary arthrodesis (12.2% vs. 0%). The highest rate of complication in surgical management group was temporary forefoot pain (55.0%).@*CONCLUSION@#The results of this study suggest that the outcomes of the surgical management with percutaneous position screw fixation are better than the conservative management to treat undisplaced subtle ligamentous Lisfranc injuries. This study can serve as a resource for orthopaedic surgeons in recognizing and managing such injuries.

ABSTRACT

Myelodysplastic syndromes are a group of malignant myeloid clonal diseases that originate from hematopoietic stem/progenitor cells.They are characterized by decreased peripheral blood cells,dysplasia of one or multiple lineages of myeloid hematopoietic cells,and prone to transform into acute myeloid leukemia.Refractory cytopenia of childhood is the most common clinical type of MDS in children.Its clinical manifestations include the decrease in one or multiple lineages of peripheral blood cells,abnormal development of the myeloid lineage,and the absence of excess blasts.This type was first introduced in WHO children's MDS in 2008.Considering the low incidence and lack of specific diagnostic methods,difficulty in early diagnosis and poor prognosis of RCC,we summarize the definition,clinical manifestations,laboratory tests,diagnosis and differential diagnosis,and treatment strategies of RCC in this review.

ABSTRACT

OBJECTIVE:To provide idea and reference for TCM therapy for hemoptysis. METHODS:The prescriptions for hemoptysis included in TCM Prescription Dictionary were screened,summarized and logged in TCM inheritance support platform software(V2.5). The medication frequency was analyzed statistically. Apriori algorithm was used to analyze combination of medicinal materials(the number of support degree was 12)and its association rules(confidence degree>0.7)statistically. Correlation coefficient of 2 different medicinal materials(related coefficient of 8,penalty coefficient of 2)and core combination of 3 and 4 ingredients were analyzed statistically by complex system entropy clustering method. Core combination and new prescription were extracted by unsupervised entropy hierarchical clustering method. RESULTS:Totally 105 prescriptions for hemoptysis were screened and included 198 ingredients,among which 39 ingredients were used more than 6 times;there were 30 common couplet medicines(including 15 ingredients)and 11 correlation regularities;48 core combination of 3 ingredients and 5 core combination of 4 ingredients were obtained. Each 6 core combination 1 and 2 were extracted for new prescription clustering to obtain 6 new prescriptions. CONCLUSIONS:The efficacy of each ingredient of the prescriptions for hemoptysis mainly include clearing heat and cooling blood,nourishing Qi and blood,eliminating phlegm and relieving cough. The efficacy of couplet medicines for hemoptysis mainly include clearing heat and cooling blood combined with nourishing Yin and blood,tonifying Qi and Yin. The composition of new prescription shows diverse characteristics,and pay attention to cooling blood to stop bleeding, nourishing Qi and nourishing Yin,or clearing heat and eliminating phlegm.

ABSTRACT

The Oncomelania hupensis snails were dually infected with Exorchis mupingensis and Schistosoma japonicum at different intervals for 21 d,37 d,55 d,70 d and 85 d.The results indicated that the development of all S.japonicum larvae were blocked in the snails of co-infection,and the complexity and number of secretions in and around all the wrecked S.japonicum larvae is proportional to the intervals of co-infection.In addition,we also described and compared the detailed change of snails' secretions in different conditions of infection,and determined that the snail's secretions may involve in the destruction and damage of S.japonicum larvae.The attack degrees on larval S.japonicum in O.hupensis snails dually infected by E.mupingensis and S.japonicum with longer intervals were stronger than that of shorter intervals,and snail haemo-lymphocytes numbers were more few in that of shorter intervals.But the secretions remarkably increased in more longer interval model experimental snail tissue.This finding may provide an alternative strategy for reducing and controlling the transmission of S.japonicum,and are very helpful for better understanding the host-parasite relationship.

ABSTRACT

OBJECTIVE: To evaluate the effectiveness of pharmacist-participated warfarin anticoagulation management by patient education and medication consultation on patients with pulmonary embolism(PE). METHODS: A total of 204 PE patients were hospitalized in respiratory wards from March 2015 to April 2017 enrolled eventually. Evaluation endpoints including the percentage of INRs within the therapeutic range(TTR), the percentage of INRs within the expanded range(TER), INR recall intervals, bleeding events, emergency department visits and hospitalizations related to anticoagulation therapy. RESULTS: TTR and TER of intervention group were(70.89 ± 26.02)% and (87.71 ± 20.01)% respectively, which were both significantly higher than (47.79 ± 22.31)% and (71.23 ± 21.47)% in control group(P = 0.000). INR recall intervals were(27.48 ± 12.81) d versus(43.35 ± 13.65) d (P = 0.000). There were no significant differences between two groups in total bleeding events(41.96% vs 29.35%, P = 0.062), minor bleeding events(38.39% vs 27.17%, P = 0.091) and major bleeding events(3.57% vs 2.17%, P = 0.864). And a significantly decreased in emergency department visits of intervention group(9.82% vs 1.09%, P = 0.008), whereas no difference was observed in hospitalizations(8.04% vs 2.17%, P = 0.125). CONCLUSION: Clinical pharmacists participated in anticoagulation management of pulmonary embolism patients, which helped patients to monitor INRs more regularly, INR controlling better and decrease frequency of emergency department visits.

ABSTRACT

Bacterial ghosts are bacterial cell envelopes devoid of cytoplasmic contents while maintaining their cellular morphology, which can be used as a new vaccine and delivery vector. In this study, a clinical isolate of avian pathogenic Escherichia coli (APEC) strain DE17 was used to prepare bacterial ghost through three different ways. The results showed that the cleavage efficiency of DE17 bacterial ghost was 99.9% with the lysis plasmid containing the PhiX174 lysis gene E. Scanning electron microscopy showed that transmembrane tunnels were formed in the middle or both ends of the cell envelope of DE17. Furthermore, the DE17 bacterial ghost was prepared with one of cell penetrating peptides (CPPs) named MAP (KLALKLALKALKAALKLA), which will completely inactivate DE17 (OD₆₀₀=0.1) by 10 μmol/L MAP. The cell envelope showed a gully-like structure and obvious transmembrane tunnels were not found through the SEM. However, the DE17 could not be lysed by importing the lysis plasmid (pBV220-MAP), which was used to express MAP. The present study will benefit for research on bacterial ghost preparation methods and provide a reference for biosafety of bacterial ghost vaccines.

ABSTRACT

Objective To explore the potential value of the virtual touch tissue quantification (VTQ) in the differential diagnosis of focal pancreatic lesions. Methods Totally 51 patients with focal pancreatic lesions underwent the quantitative analysis by VTQ. Based on the pathologic or clinical diagnosis,the VTQ values including lesional shear wave velocity (LSWV),parenchymal shear wave velocity (PSWV),and difference shear wave velocity (DSWV)(DSWV=LSWV-PSWV) were compared between the lesions and background parenchyma. Results The mean LSWV and PSWV were (2.39±1.25) m/s(0.60-4.39 m/s) and (1.59±0.63) m/s (0.76-3.22 m/s) in malignant group,(1.92±1.07) m/s(0.79-4.00 m/s) and (1.43±0.41) m/s(0.80-2.23 m/s) in potentially malignant group,and (2.40±1.10) m/s (0.89-3.42 m/s) and (1.48±0.44) m/s (1.03-2.11 m/s) in benign group. There were statistically significant difference between LSWV and PSWV in bengin and malignant group (P=0.029,P=0.005),while no statistical significance in potential malignant group (P=0.087). However,LSWV,PSWV,and DSWV showed no significant difference among these three groups (P=0.401,P=0.638,P=0.625,respectively). LSWV was not significantly associated with the tumor size (r=0.253,P>0.05) but had negative correlation with the depth of region of interest (r=-0.413,P<0.05). Conclusion VTQ may be valuable in the differential diagnosis of focal pancreatic lesions,although further research is still required.

ABSTRACT

Uric acid is the final end-product of purine metabolism and 70% of it is excreted through the kidneys. Chronic hyperuricemia can easily cause gout and meanwhile may affect the kidneys and cardiovascular system. In recent years,the incidences of hyperuricemia and gout have gradually increased. This article summarizes the correlation of hyperuricemia with hyperuricemic nephropathy,gouty arthritis,non-alcoholic fatty liver disease,and carotid atherosclerosis and the role of color Doppler ultrasound in the diagnosis of these lesions.

ABSTRACT

OBJECTIVE:To systematically evaluate therapeutic efficacy of Xuefu zhuyu decoction in the adjuvant treatment of unstable angina pectoris (UAP),and to provide systematic evidence-based reference in clinic.METHODS:Retrieved from PubMed,EMBase,Cochrane library,CJFD,Wanfang database,VIP and CBM,randomized controlled trials (RCTs) about Xuefu zhuyu decoction+conventional treatment (trial group) vs.single conventional treatment (control group) in the treatment of UAP were collected.Meta-analysis was performed by using Rev Man 5.2 statistical software after data extraction and Cochrane systematic review manual 5.1.0.RESULTS:Totally 12 RCTs were included,involving 1 252 patients.The results of Meta-analysis showed that total response rate [OR=3.56,95%CI(2.49,5.10),P<0.001],serum HDL[WMD=0.25,95%CI(0.05,0.45),P=0.01] and the rate of ECG improvement [OR=2.76,95%CI(1.97,3.87),P<0.001] of trial group were significantly higher than those of control group,while serum TC [WMD=-1.14,95%CI(-1.46,-0.82),P<0.001],TG [WMD=-0.53,95%CI(-0.84,-0.22),P<0.001] and CRP [WMD =-0.91,95 % CI (-1.14,-0.69),P< 0.001] levels of trial group were significantly lower than those of control group,with statistical significance.CONCLUSIONS:Therapeutic efficacy of Xuefu zhuyu decoction in adjuvant treatment have good therapeutic efficacy and can significantly improve the blood lipid and inflammatory factors level.
